The Referral Process
- Contact the practice to discuss the evaluation.
- Confirm whether the requested assessment is appropriate and available.
- Agree on the location, referral documents and appointment arrangements.
- Complete the agreed assessment.
- Confirm reporting arrangements and authorised recipients.
Assessment findings and recommendations depend on the information available and the assessment undertaken.
